Terms & Conditions:
1.This campaign is valid for purchases of Pristin, Lacto-5, Narcox, Bitreen, Moo, Shartilage, Nutracare and Nugevity products made between 1st October 2024 and 31st December 2024, and is only valid for purchases in same pharmacy.
2.The campaign only valid for physical retail pharmacy transaction.
3.Customers must provide complete details and upload the original purchase receipts via the provided link.
4.Each valid receipt entitles the participant to redeem once.
5.The campaign is open to residents of Malaysia aged 18 and above.
6.Participants must submit their purchase receipts via Google Form before 5th Jan 2025.
7.Any incomplete, illegible, or fraudulent submissions will be disqualified.
8.Each valid HP number can only redeem 1 freegift.
9.The prize will be courier to the address submitted before 31st Jan 2025 after verification by HOLISTA.
10.The rebate is non-transferable, non-refundable, and cannot be exchanged for cash.
11.HOLISTA shall not be responsible for any technical issues or failures that may affect the submission or redemption process.
